What numbers are you trying to call in those two countries - landlines, mobiles, or other types of numbers? Can you list examples of numbers in both countries? If so, please replace the last 4 digits of any number you list with XXXX, but provide the other digits. This can help in confirming what type of number it is. In some countries, there are special numbers that can only be called using Skype Credit, classified as "shared cost" or "special services" that are distinct from "landline" and "mobile" phones.
If you're able to call numbers in the US, it sounds like you might have a subscription for calls to the US (possibly the "Unlimited USA & Canada" subscription), but nothing to cover calls to those European countries. If this is the case, you would either need a subscription to cover those other calls. You can get additional subscriptions for calls to other countries, or look at something like the Unlimited World plan (this would cover calls to landline phones in many European countries, but not mobile phones - but covers calls to landlines and mobiles in the USA and Canada). Or add Skype Credit to your account, so those calls are paid for as you make them.

The Skype app is trying to be "helpful", based on where you are normally located, by preloading the +1 into the dialing pad. One way to get around that is just backspace over the 1, leave the +, and then dial the country code and the rest of the number you want to reach.
For your contact list - whether in Skype or your iPod Touch (same for iPhones and iPads) is to store the + and country code in each number you have in your contact list. Even if the numbers are in the US, where you would have a +1 before every area code and phone number in there, this ensures Skype could dial the number without delay. The 011 is the international access code used in the US, Canada, and other countries/territories in the +1 country code to place an international call, but from Skype or mobile phones using the + is a more universal way to place the call.
