I bought $14 in credit last month and have just 'purchased' another $3 without my knowledge or authorization. The trouble is, when I check my call history, all my calls says '*Amount $0.00'. The original $14, plus the new $3 should give me $17. In Skype-land, I only have $8 in credit. Why is that?

My immediate concern is that your accounts may have been compromised or "hacked." Please change your account passwords immediately, as well as that of the payment method on file: if you process payments via PayPal, please report these incidents to PayPal as well as the issuer of the payment method you use to fund your PayPal accounts.
Please also perform a maintenance cycle on your computer to rule out an inadvertent malware infection. Download and install system updates and patches. Run your preferred anti-virus/malware/spyware program using the lates virus signature files available.
